- Your home kit arrives at your house within 10 days.
Everything you need to collect vital health information is included in the kit. And the blood pressure monitor is yours to keep. - Take your measurements
Check your blood pressure, measure your abdominal circumference and stick your finger with the easy-to-use spring-loaded lancet. - Mail your results to the lab
Pack everything up in the postage PAID envelope and drop it in the mail. - Schedule an appointment.
You’ll be notified when your lab work is complete to schedule an online appointment with a Catapult nurse practitioner. - Complete a health questionnaire
Before your appointment with the nurse practitioner you’ll answer a few questions about your health history and habits. - Review your results and develop and action plan
Your private online nurse practitioner appointment will review results and develop any necessary action plans.
